Importing forms
To build the registry, i.e., add new or modified forms to the registry, return to the registry design page and refresh the list of available forms by clicking Refresh list. Import forms into the registry by dragging and dropping them from the list on the right to the upper (basic data forms) or lower (longitudinal data forms) left area. Basic data forms refer to forms that can be completed once per patient, e.g., for basic patient information or an initial family history. Longitudinal data forms, on the other hand, will be available within episodes and can be completed several times, usually associated with a specific point in time, e.g., for repeat examinations or laboratory analyses during regular patient visits.
Selecting forms for the registry
A single form can only be imported into the registry once; a form cannot exist as a basic and longitudinal data form at the same time. Forms can be removed from the registry by clicking the bin icon while the order of forms in each section can be adjusted by selecting a form and dragging it into the correct position.
Once all forms have been added to the registry click Save changes; alternatively the Cancel button can be used to undo any changes. After saving the changes, a pop-up message in the top right corner of the page will confirm that the forms have been imported and are available for use in the OSSE.EDC.
Please note that any changes to registry forms made in the FormEditor will not automatically appear in the forms currently available in the registry. After modifying an existing form, the old version has to be manually replaced with the current version by dragging the current version into the area on the left.
